How do you report a daycare that is unclean and unhealthy??

i'm working at a daycare for the first time ever and i thought it would be a better experience, the lady is off her rocker and doesn't know what she is doing. The place is unclean and the kitchen were the meals are prepared are, is dirty, moldy, etc. How do i report this? I have no clue what number to call. I even taken pictures. The children don't seem to get any structure and i feel something should be done. I guess i'll risk my job just so these kids can get somewhere better. The floors are dirty and only swept on weekends. It is horrible. Everyone there agrees but nobody knows what to do. Please help!!!!!!!
